Jordan Valley Medical Center maintains medical records for each patient treated at the hospital.

To request a copy of your medical records, please contact our Health Information Management office between the hours of 7:00 a.m. and 4:30 p.m. Monday through Friday at 801-562-4227.

Prior to receiving your records, you will be asked to complete a medical records release authorization form and to provide proof of your identification with a valid government issued photo ID.

Records needed for continuing care purposes will be sent directly to your provider.

Requests for medical records generally take up to 72 hours to process.
